Patient Preparation and Counseling

Medical Art Center recognizes that dealing with fertility or high-risk pregnancy can be very stressful. We therefore give emotional support and counseling during these difficult times. This includes a thorough evaluation of the detailed questionnaire, which has been designed to bring out all the possible factors that may be causing the infertility. Statistics are used to make the patient know they are not alone in their problems. For instance, the fact that twenty-five prevent of couples in their thirties are infertile and only 1 percent of teenagers are, create a not so lonely picture.

Endocrine Management of Infertility

There are several hormonal causes of infertility. The process of ovulation and pregnancy are under the influence of the brain, hypothalamus and the pituitary glands. These three centers produce substances called hormones and growth factors as well as induction factors that act on the ovary and the uterus to produce the net effect of ovulation, implantation and subsequent pregnancy. In addition, some other organs play a significant modulation on this process; they include the adrenal gland, the thyroid gland, the pancreatic islet of Langerhans to mention a few. However, all these hormones and growth factors operate like an orchestra in a symphony. If any single one should play out of tune, the net effect can be very significant and lead to infertility. We have qualified and experienced endocrinologists to carry out evaluation of the endocrine panels and treat accordingly.

Transvaginal Ultrasound

Check the following:

  • Ovaries: size, Volume, Position, Shape, Cyst, Basal antral follicle count and evidence of PCO.
  • Uterus: Endometrial size, shape & texture, polyp, fibroids & congenital anomalies.
  • Hysterosonogram: it is a procedure that allows the inside of the uterus (endometrial cavity) to be seen and inspected for abnormalities such as polyps, submucus fibroids and adhesions. It is not a painful procedure; it lasts 10 – 20 minutes. Sterile saline is injected through the cervix with a catheter; you may experience mild cramping or a little discomfort during the procedure. It is done between days 5 – 12 of the cycle.
  • Hydrosalpinges or loculated fluid in the tubes.

Super Ovulation

 

Fertility drugs are given to number of follicles in ovary.

Male Assessment

 

  • Semen analysis to determine the count, motility and morphology of the average sperm after 2 – 3
    days abstinence.
  • A blood evaluation is done for HIV 1&2, syphilis, Hepatitis B, Hepatitis C, blood group and
    genotype.

 

Endocrine & Blood Evaluation

 

  • Blood is drawn between day 5 – 12 of the menstrual cycle
  • Full blood count

Normal values should range between the under listed tests:

  • FSH: <10miu/ml
  • LH: FSH/LH:<3
  • Prolactin: <25ng/ml
  • TSH: <6.5mu/l

Preimplantation genetic testing is a molecular genetics examination used to determine if genetic or chromosomal disorders are present in embryos produce by injection (ICSI). This genetic screening ensures that embryos selected for transfer into the woman’s uterus are free of chromosomal abnormalities, thereby increasing the chances of a successful pregnancy. What Types Of Couples Are Candidates of PGT?

PGT is recommended for the following people:

  • Advanced maternal age
  • History of recurrent miscarriages
  • Family balancing using sex selection
  • Sex-linked genetic disorder such as hemophilia
  • One of the parents is a carrier of chromosome translocation
  • One of the parents is a carrier od chromosomal inversion
  • Both parents are carriers of a sickle cell gene.

Since numerical chromosomal abnormalities are common cause sin most unsuccessful pregnancies, screening for aneuploids (abnormal chromosomes) in human embryos increases the chances of successful IVF treatment.

Chromosome Analysis: Karotyping

karyotype analysis is a specialized genetic test performed using peripheral blood to detect chromosomal anomalies that may be associated with infertility. Results from karyotype analysis can assist IVF physicians to rule out genetic factors and provide appropriate therapy in achieving a successful pregnancy.

Oocytes and Embryo Donation

This procedure is available for patients with premature ovarian failure. We encourage a number of our successful patients to donate their embryos if they no longer have a need for it. We also have a successful egg donation program.

Uterine Receptivity and Embryo Transfer

One of the areas we have been able to make significant changes in our pregnancy rate is our devotion to the problem of implantation defect and uterine receptivity. We evaluate our patients and ensure we eliminate several implantation defects, increase the receptivity of the uterus for the embryos and support the early phase of development.
